"Handedness", modifiers and remapping

I've been noticing more and more recently how much I'm moving my hands while typing, and am starting to think about where they are moving to and from, particularly since hurting my left hand a few days ago. I'm using a Filco tenkeyless with a Kensington Expert nuzzled up against the right, and am right handed. For most text work I can keep my wrists pretty much stationary, but as soon as I need modifiers (singly or in combination) I move my hands around awkwardly. I also notice I tend to use my left hand for modifier combinations, leaving the ones at the right of the keyboard pretty much redundant.

I'm using OSX almost exclusively, and am aware of a few remappers. Really though I'm interested in knowing how people have arranged their own modifiers and why, and how the changes make a difference. Are there more optimal placements for these, or is the default the most ergonomic all-rounder?
